The road coming in from bottom left is the A961 from Kirkwall which continues, in the same direction at the junction just under the aileron, south to St Mary's [just over the leading edge of the wing to the right] and the southern islands. The road going straight ahead into the distance is an unclassified road to Netherton and Hurtiso. The small island is Lamb Holm, joined to Mainland by Churchill Barrier No.1 - see HY4801. Two aerials are just visible in the picture at the bottom. These do not appear to exist now, but the base of one is shown in 1340189 and identified as a WW2 radio station; see also other comments in images in HY4604.
