Past Medical History: Fibromyalgia, Neuropathy, Palpitations, Rheumatoid Arthritis, Hysterectomy Social History: Smoker (unknown quantity) Allergies: Ceclor, Neurontin, Chantix

Medication List • Methotrexate 2.5mg, 7 tabs every Wednesday • Folic Acid 1mg daily • Cyclobenzaprine 10mg TID • Toprol XL 150mg daily • Cardizem CD 120mg daily • Fioricet 1-2 tabs every 6 hours PRN pain (MDD = 6 tabs) • Percocet 10/325, 1 tab every 4 hours (MDD = 6 tabs) • Nexium 40mg daily • Mirapex 2.?mg daily A Drug Without Indication • Mirapex (pramipexole) 2.?mg daily Indications: Parkinson’s Disease: 0.5-1.5mg TID Restless Leg Syndrome: 0.125-0.5mg 3h before bed Depression (off-label) Available Doses: 0.125, 0.25, 0.5, 0.75, 1, 1.5mg & 0.375, 0.75, 1.5, 3, 4.5mg ER Mechanism of Action: Unknown, but believed to stimulate Dopamine receptors in the striatum Side Effects: Malignant Melanoma, Sleep Attack, Orthostatic Hypotension, Amnesia, Hallucinations, Impaired Impulse Control, Dyskinesia Pregnancy Category: C Cautions: Avoid abrupt dose reduction or discontinuation. This may cause hyperpyrexia (>106o) and confusion, similar to neuroleptic malignant syndrome. T1/2: 8 hours
• The patient does not have any FDA approved indication or off-label indication warranting the administration of Mirapex. If the patient was administered Mirapex, there is a risk of serious side effects, including Malignant Melanoma, Hallucinations, and Impaired Impulse Control. Also, when discontinuing Mirapex therapy, there is a risk of hyperpyrexia and confusion.
P Remove Mirapex from drug list. Since it was never administered, there is no discontinuation risk.
