Your edits claimed that an individual was responsible (in two cases - and they were different individuals in the different edits), and the third held the "staff" responsible. None of the cited sources made any of those claims, and in fact all they said was that an investigation is planned. In the case of Swarbrick in particular, to claim that the AD is responsible for an individual action regarding an individual person is an unusual claim to say the least.
