The article states that usted (formal) is used instead of tú (informal) upon initial meetings. However, usted is universally used in formal situations and it is vos (informal) that is used instead of tú (informal) in several countries including Costa Rica. Am I wrong here?

Somewhat...in some parts of Costa Rica it's rare to use "tú" or "vos" and "usted" is used for second person regardless of who you are talking to. While you are right on the traditional use of this in formal and informal conversations, Costa Rican Spanish is the exception in this.--
